Charity Specimen Forms

Each of the following, which are generally functioning Excel files, may be downloaded. Those marked with an asterisk may be amended for use by individual Lodges. Specimens of the other charity-related forms may be found in the Charity Stewards’ Manual.

Annual Charity Accounts *

This is an example of the Annual Charity Accounts which should be presented to members at a Regular Lodge Meeting each year before being sent to the Provincial Office, within 3 months of the end of the accounting year.

Cash book *

This is an example of a cash book which should be kept to record all Charity transactions. It is a transparent record of all Charity payments both in and out, and shows if the transaction was cash or cheque.

Donations to Charities Form *

This form is completed by the Charity Steward every time he sends donations to Province. It should show the individual’s contribution and indicate if tax is to be reclaimed or not, and where the Brother wishes the payment to go to. A separate form is required for each Charity.

Member’s Payment Record *

This is an example of the payment record which should be kept for each donating Brother and which can be downloaded for use.

Jewel Application Form

Charity Stewards may apply on behalf of qualified brethren for a Festival Jewel using this application form. The qualifying amount, £100 excluding gift aid, must have been received by the Provincial office before the application is made; it is not sufficient that the amount has been received on the Lodge Charity account. The Jewel may be presented in Open Lodge if the recipient agrees to this form of presentation.

Gift aid form

Gift Aid is reclaimed by the charity from the tax you pay for the current tax year. Your address is needed to identify you as a current UK taxpayer.
