This form is completed by the Charity Steward every time he sends donations to Province. It should show the individual’s contribution and indicate if tax is to be reclaimed or not, and where the Brother wishes the payment to go to. A separate form is required for each Charity.
This is an example of the payment record which should be kept for each donating Brother and which can be downloaded for use.
Charity Stewards may apply on behalf of qualified brethren for a Festival Jewel using this application form. The qualifying amount, £100 excluding gift aid, must have been received by the Provincial office before the application is made; it is not sufficient that the amount has been received on the Lodge Charity account. The Jewel may be presented in Open Lodge if the recipient agrees to this form of presentation.
Gift Aid is reclaimed by the charity from the tax you pay for the current tax year. Your address is needed to identify you as a current UK taxpayer.
